OCEDS Events Reporting
This should be used if a member of the Collaborative attends an event that applies to the overall goals of the Collaborative – this form will go directly to K. Pickering upon submission and should be sent as soon as possible after the event.
OCEDS Monthly Reflections
This form should be used to reflect on the monthly contacts/work with the Oweesta Collaborative and EDS related activities – this form will go directly to K. Pickering upon submission, who will use it for evaluation/reporting purposes.
Program Highlight / Success Story
This template is to be used to document individual program highlights/success stories that each partner of the Collaborative has been involved with. Examples of highlights/stories include engaging in client loan/business start-up, starting a new business course, presenting at a conference, changing policy or local ordinance to support entrepreneurship, etc. These are submitted electronically to S. LaCroix on at least a biannual basis and will be available to the public on the site.
Business Resource
This template will be used for each Business Resource submitted by the partners of the Collaborative. This can be any resource or potential mentor that offers assistance with business plans, marketing, accounting, management, etc.; or these individuals may have very specialized skills that can help other OC members assist their clients.
Initial Interview
Success Coaches use this form to begin to understand the needs, desires, and experience levels of a client so that the Coach then knows how best to help the client succeed in business. The form is a reminder to ask about each of the interests and desires of a client and to assess the knowledge that the client will bring to his or her business endeavors. After a Success Coach assesses the client, the form also helps categorize and aggregate clients for compiling reports that show each Coach's effectiveness and productivity. Reporting helps the Collaborative know, for instance, how many jobs are to be created by clients or how many new businesses are starting up.
Servicing Report
The Servicing form is used by Coaches to keep track of the details of each client meeting. If there is a change in contact information, Coaches capture it here. When a Success Coach works with a client one on one, they share some of the responsibilities for the next step in the unfolding process of growing a business. This form helps each know what those responsibilities are and who will be doing which thing. The form also helps in updating reports for Coach performance.
Annual Report: Progress Towards Goals
This form will provide detail regarding progress toward intended goals during the grant year. Respond to the specific evaluation questions for your project as outlined. This form will be send to K. Pickering and M. Utter upon submission.
Progress Update Report to CFED
The purpose of this report is for CFED to track the progress and lessons learned of each of the W.K. Kellogg EDS grantees for three reasons: Facilitate peer exchange and knowledge building among the six collaboratives, Document learnings and outcomes across the sites for the Foundation, the grantees and the larger field, Provide technical assistance to grantees as they face challenges in their systems development and workplan implementation. This form will be sent to K. Pickering and M. Utter upon submission.
